OLYMPIA – A Health Law Judge issued an indefinite suspension for the license of Armstrong Alcohol and Drug Recovery, LLC (BHA.FS.60873835).
The initial charges issued in August 2024 allege that Armstrong Alcohol and Drug Recovery, located at 625 Strander Blvd., Ste C, Tukwila, WA 98188-2900, had previously failed to cooperate with the Department of Health by not submitting a plan of correction (POC) in response to a statement of deficiencies (SOD) issued for violations of licensing standards identified during a routine inspection. The facility was provided with several opportunities to address deficiencies, but did not provide a POC to remedy those deficiencies.
