State disciplines health care providers

OLYMPIA -- The Washington State Department of Health has taken disciplinary actions or withdrawn charges against the following health care providers in our state.

The department’s Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Information about health care providers is on the agency website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov).The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are encouraged to call and report their complaint.

Clark County

In March 2023 the Podiatric Board charged podiatrist Stephen M. Hayes (PO00000795) with unprofessional conduct. The charges state that in 2017, Hayes improperly billed patients eight times.

Cowlitz County

In March 2023 the Nursing Commission charged Melissa L Gibson (RN00160154) with unprofessional conduct. In 2016, Gibson was denied a registered nurse license in Oregon after finding that in 2014 Gibson was convicted in Washington state for disorderly conduct-domestic violence and obstructing a law enforcement officer. Then, in 2018, Gibson was convicted in Oregon of menacing and attempt to commit a felony with a weapon after threatening her daughter’s boyfriend and family members with a knife.

Grays Harbor

In March 2023 the Dental Commission granted with conditions the dental assistant license of TreAnn Louise Andrew (D161384589). Andrew agreed to the conditions that require she enroll in and complete a substance use monitoring program. In March 2015, Andrew was convicted of driving under the influence and a hit and run. In September 2020, Andrew was convicted of assault, and in March 2021 for driving under the influence.

King County

In March 2023 the Medical Assistant Program charged phlebotomist Chelsie Marie Andrew (PC60346351) with unprofessional conduct. In September 2020, Andrew was convicted of assault and malicious mischief in relation to two 2019 incidents of physically attacking someone while intoxicated.

In March 2023 the secretary of health ended conditions on the substance use disorder professional trainee license of Jeannette Lorinda Asher (CO60883669).

Kitsap County

In March 2023 the Nursing Commission charged registered nurse Caprice Carole La Dieu (RN60409110) with unprofessional conduct. The charges state that La Dieu did not comply with a 2022 condition to her license that she complete a refresher course and continuing education requirements.

Okanogan

In March 2023 the Nursing Commission charged registered nurse Colleen Marie Dorsten (RN00176717) with unprofessional conduct. Charges state that in June 2022, Dorsten took a child in the custody of the Department of Children, Youth & Families to Uganda and has not returned.

Pierce County

In March 2023 the Occupational Therapy Practice Board denied the occupational therapy assistant limited application of Ricky Pheth (LA61222624) because Pheth did not respond to proposed conditions on the license. Pheth was convicted of driving under the influence in 2011 and 2019.

In March 2023 the Veterinary Board of Governors charged veterinarian Joel Dennison Cuthbert (VT00007068) with unprofessional conduct. Charges state that in 2019, Cuthbert provided consulting services to an animal rescue wherein he recommended treatments that were outside the scope of what the rescue could perform without supervision. Cuthbert allegedly failed to maintain records of these consults and of a procedure he is alleged to have performed elsewhere.

In March 2023 the Substance Use Disorder Program withdrew the denial of credential against substance use disorder professional trainee Jennifer Margarett Richards (CO61267275) and granted Richards’ credential subject to conditions. Richards will be on supervised probation for three years and must submit quarterly performance evaluations. In 2019, Richards was convicted of burglary and of theft in the second degree.

In March 2023 the secretary of health withdrew the cease-and-desist order against Haylo Care Inc. doing business as Brightstar Care.

Snohomish County

In March 2023 the secretary of health issued a cease and desist to Tyneshia Charisma Alexander. Alexander must pay a $2,000 fine and permanently end all practice unless she receives appropriate licensure. Alexander practiced as a naturopathic physician and midwife but doesn’t have and never has held a license for either.

In March 2023 the secretary of health ended the conditions on the medical assistant phlebotomist license of Saniza Neaha Nisha (PC61233363).

Spokane County

In March 2023 the Nursing Commission charged registered nurse Patricia L. Morrison (RN00130481) with unprofessional conduct. The charges state that in October 2022, Morrison tested positive for alcohol, which makes her out of compliance with a required 2020 substance monitoring program.

Stevens County 

In March 2023 the Nursing Commission charged registered nurse Chad McLain (RN60736025) with unprofessional conduct. Charges state that in June 2022, while working on a unit with positive COVID-19 cases, McLain cut slits into the side of his N95 mask. McLain did not respond to requests for a statement.

Whitman County

In March 2023 the secretary of health ended the conditions on the home care aide license of Jackie Dawn Fry (HM61278593).

Out of State

California: In March 2023 the Unlicensed Practice Program notified Raghdah Al-Baytari of its intent to issue a cease-and-desist order. Between December 2021 and January 2022, Al-Baytari is alleged to have within the state of Washington, given five people aesthetic injections for which he received payment. In December 2021 and July 2022, Al-Baytari’s social media profiles updated claims he is a medical aesthetician and showed images of clients. Al-Baytari does not and has never had a physician license in Washington.
