State disciplines health care providers

OLYMPIA -- The Washington State Department of Health has taken disciplinary actions or withdrawn charges against the following health care providers in our state.

The department’s Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Information about health care providers is on the agency website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov).The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are encouraged to call and report their complaint.

Cowlitz County

In June 2023 the Nursing Assistant Program charged certified nursing assistant Bradley Norman Laird (NC61222933) with unprofessional conduct. Charges state that in August 2022, Laird entered into a romantic and sexual relationship with a patient of the assisted living facility where he worked.

In June 2023 the Nursing Assistant Program charged certified nursing assistant Angela M. Lasseigne (NC10060601) with unprofessional conduct. Charges state that in December 2021, Lasseigne took $300 from a patient at the adult family home where she worked.

Island County

In June 2023 the Nursing Assistant Program charged registered nursing assistant Kayla Erin Wilson (NA60049975) with unprofessional conduct. Charges state that in February 2021, Wilson signed a five-year referral to a substance use monitoring program in lieu of disciplinary action. In June 2021, Wilson was discharged from the monitoring program for allegedly missing check-ins, testing, and failing to respond to communications. In December 2021, a different license of Wilson’s was suspended. In May 2022, Wilson was allegedly arrested for driving under the influence, a gross misdemeanor.

King County

In June 2023 the Nursing Assistant Program charged certified nursing assistant Alex Mwangi Ngatia (NC60445961) with unprofessional conduct. Charges state that in December 2022, Ngatia pleaded guilty to a charge of fourth-degree assault – domestic violence, a gross misdemeanor.

In June 2023 the secretary of health issued a cease-and-desist order to Fei Li. In October 2022, Li agreed to massage a Department of Health investigator in exchange for payment. Li does not and has never held a credential to practice as a massage therapist in the state of Washington. Li must not practice massage therapy in Washington and must pay a $1,000 fine.

Kitsap County

In June 2023 the Nursing Commission ended probation on the practical nurse license of Stella Marie Guerrero (LP00040764).

Lewis County

In June 2023 the Dental Quality Assurance Commission and dentist John Pham (DE00008608) entered an agreed order. The license is restricted for four years, during which time Pham can only perform up to three simple dental extractions at a time and cannot perform surgical extractions unless a Commission-approved proctor is present. The proctor must submit quarterly performance evaluations and will advise the Commission if additional years of compliance are needed. An investigator will audit patient records at least twice a year for four years. In May 2021, an investigator found that Pham performed unsupervised surgical extractions in violation of an order Pham entered in March 2020.

Pierce County

In June 2023 the Unlicensed Practice Program notified Nikilauni Rae Simon-Lovett of its intent to issue a cease-and-desist order. Between February 2020 and November 2022, Simon-Lovett is alleged to have provided lip filling services using a non-FDA approved filler. Simon-Lovett does not and has never held a physician credential in Washington state.

In June 2023 the secretary of health issued a cease-and-desist order to Shuhua An. In February 2022, An offered to provide massage to an undercover police officer in exchange for $40. An does not and has never held a massage therapist credential in the state of Washington. An must not practice massage in Washington and must pay a $1,000 fine.

Snohomish County

In June 2023 the secretary of health withdrew an intent to issue a cease-and-desist order against Janelle Cass.

In June 2023 the Nursing Commission ended conditions on the registered nurse license of Anna Christina Strickland (RN60405937).

In June 2023 the secretary of health and Christina L. Audet (CM61417361) agreed to conditions on her certified medical assistant license. The license will be on probation for one year during which time Audet’s employer must submit quarterly performance evaluations. In October 2003, Audet entered into an agreed order that revoked her pharmacy technician license with no right to reapply for 10 years. In February 2014, Audet failed to comply with an agreement to participate in a substance use monitoring program and subsequently surrendered her registered nursing assistant license.

Spokane County

In June 2023 the secretary of health and Douglas Lewis Enos (XT61306982) agreed to conditions on his x-ray technician license. Enos must enroll and participate in a substance use monitoring program. In June 2015, Enos was convicted of driving under the influence, a gross misdemeanor; in August 2017 of ignition interlock device violation, a gross misdemeanor; in January 2020 of driving under the influence, a gross misdemeanor; in November 2021 of physical control, a gross misdemeanor; and in March 2022 of third-degree theft, a gross misdemeanor.

Whatcom County

In June 2023 the Home Care Aide Program charged Mervyn Arnold Fortune (HM60581912) with unprofessional conduct. Charges state that Fortune was responsible for $6,000 in charges to the personal bank account of a resident at the facility where Fortune worked.

Yakima County

In June 2023 the secretary of health ended conditions on the substance use professional disorder trainee license of Cristin Dawn Georgeton (CO60891063).

Out of State

California: In June 2023 the secretary of health issued a cease-and-desist order to Raghdah Al-Baytari. Al-Baytari must not practice medicine in the state of Washington and pay a $1,000 fine. Between December 2021 and January 2022, within the state of Washington, Al-Baytari gave five people aesthetic injections for which he received payment. In December 2021 and July 2022, Al-Baytari’s social media profiles updated claims he is a medical aesthetician and showed images of clients. Al-Baytari does not and has never had a physician license in Washington.

Idaho: In June 2023 the Nursing Commission reinstated the licensed practical nurse license of Amy K. Ross (LP00050636) subject to probation. Conditions require that Ross complete a refresher course, continue to comply with a substance use monitoring program, and complete eight hours of continuing education in the area of ethics.
