OLYMPIA -- The Washington State Department of Health has taken disciplinary actions or withdrawn charges against the following health care providers in our state.
The department’s Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.
Information about health care providers is on the agency website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov).The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are encouraged to call and report their complaint.
Benton County
In November 2023 the secretary of health placed the massage therapist license of Kristin Ann Kayfes (MA60689986) on probation with conditions. The license is on probation for five years, during which time Kayfes must complete six hours of continuing education in the area of substance misuse or abuse. She must also enroll and participate in a monitoring program and pay a $1,000 fine. In October 2021, Kayfes plead guilty to fourth-degree assault in Benton County Superior Court.
In December 2023 the secretary of health released Kathleen Marie McIntosh (CB61249067) from the terms and conditions on her certified behavior technician license.
Clark County
In December 2023 the Pharmacy Quality Assurance Commission released Debra Sue Seitz (VB61161192) from the terms and conditions on her pharmacy assistant license.
King County
In December 2023 the secretary of health terminated the probation on the certified nursing assistant and health care assistant license of Caldera W. Fries (HC00141666, HC00150777, HC00152116, NC10047803).
Kitsap County
In November 2023 the sectary of health denied the home care aide and home care aide provisional certification licenses of Brook Nikcole Cissney (HM61213960, PV61214011). In March 2013, Cissney was convicted of second-degree identity theft and bail jumping, both class C felonies. In 2018 and 2023, Cissney did not comply with notices of required substance use evaluation.
Lewis County
In December 2023 the secretary of health released Teresa Jean Slighte (NC10019123) from the terms and conditions on her certified nursing assistant license.
Pierce County
In November 2023 the Board of Nursing (formerly the Nursing Commission) reinstated the licensed practical nurse license of Stephen Wakibi Buteraba (LP60319984) with conditions on the license. The license is on probation for at least 18 months, during which time Buteraba’s practice must be under indirect supervision and he must not be the head nurse or charge nurse, or direct other nurses’ work. Buteraba’s employer must submit quarterly performance evaluation reports.
In December 2023 the secretary of health terminated the probation on the agency affiliated counselor license of Charnay Dibble Ducrest (CG60856429).
In December 2023 the secretary of health released Kianna Rose Martinez (NC61348485, NA61318110) from the terms and conditions on her certified nursing assistant and registered nursing assistant licenses.
Snohomish County
In December 2023 the secretary of health terminated the probation on the substance use disorder professional trainee license of Adnan Saim Kaya (CO61316941).
In December 2023 the Home Care Aide Program charged Romulo Ramos De Guzman (HM61053327) with unprofessional conduct. De Guzman failed to respond to a letter of cooperation regarding complaints of sexual misconduct of two patients in an assisted-living facility.
In December 2023 the secretary of health reinstated the respiratory care practitioner license of Rebecca Lynn Rose (LR00002210) without conditions on the license.
Spokane County
In December 2023 the secretary of health reinstated the registered nursing assistant license of Amanda Marie Boisjolie (NA00189023) without conditions on the license.
Thurston County
In December 2023 the Nursing Assistant Program reinstated the certified nursing assistant license of Sirina Teouri (NC60605553) with conditions on the license. The license is on probation for at least one year, during which time Teouri’s employer must submit quarterly performance evaluation reports.
Whatcom County
In December 2023 the secretary of health terminated the probation on the substance use disorder professional, agency affiliated counselor, and mental health counselor associate licenses of Kelly Ryan Frost (CP60545681, MC61341676, CG61270543).
In December 2023 the Massage Therapist Program charged massage therapist Kyndal Christopher Kreiser (MA61311675) with unprofessional conduct. Charges state that Kreiser inappropriately touched a patient while providing her a massage.
Yakima County
In December 2023 the secretary of health terminated the probation on the substance use disorder professional trainee license of Shannon Kristine Rosales (CO61197813).
Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
