State disciplines health care providers

OLYMPIA -- The Washington State Department of Health has taken disciplinary actions or withdrawn charges against the following health care providers in our state.

The department’s Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Information about health care providers is on the agency website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov).The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are encouraged to call and report their complaint.

Cowlitz County

In January 2025 the secretary of health denied the application of home care aide Amy Lenae Sanders (HM61069871). In December 2022, Sanders was found on multiple occasions sleeping in their car during their work hours while assisting a vulnerable adult. In April 2024, Sanders also financially exploited a vulnerable adult.

In January 2025 the Dental Quality Assurance Commission charged dentist Tatiana Trott (DE60135332) with unprofessional conduct. Alleged facts state that in January 2023, Trott failed to follow cough etiquette, safe injection and sharps safety, and sterilization and disinfectant procedures in her dental office during an unannounced infection control inspection. During the inspection, Trott also failed to have an assistant present during a patient’s procedure and requested a patient’s assistance with taking x-rays without the patient holding a dental or medical assistant license. Trott’s patient records also indicated a pattern of incomplete or inaccurate documentation, unnecessary treatment, fraudulent billing and treatment below the standard of care.

Grant County

In January 2025 the secretary of health charged certified nursing assistant Amanda Jean Pelot (NC60725186) with unprofessional conduct. In May 2024, Pelot was charged with 61 counts of second-degree identity theft – accomplice and two counts of second-degree possession of stolen property – accomplice.

Kitsap County

In February 2025 the secretary of health terminated the probation on the agency affiliated counselor license of Angela Marie Piper (CG61324469).

Pierce County

In January 2025 the secretary of health withdrew the charges against agency affiliated counselor and mental health counselor associate Sabrina Lea Danley (CG61069835, MC61245664).

In February 2025 the secretary of health granted Melissa Ann Sams’ (CO61607055) substance use disorder professional license with conditions. The license is on probation for at least four years. During this time, Sams must comply with the 2023 agreement to practice with conditions. Sams’ health care supervisor must also submit quarterly performance evaluation reports. In March 2019, Sams was convicted of attempted unlawful possession of a controlled substance with intent to deliver – amphetamine/methamphetamine, a class B felony. In May 2022, Sams was convicted of possession of a controlled substance – amphetamine/methamphetamine, a class B felony.

In February 2025 the Pharmacy Quality Assurance Commission reinstated Chelsea L. Tallman’s (VA00046871) license to practice as a pharmacy technician with conditions. The license is on probation for at least five years, during which time Tallman’s health care employer must submit quarterly performance evaluation reports. Tallman must seek a substance abuse evaluation through a recovery assistance program.

Snohomish County

In February 2025 the Dental Quality Assurance Commission charged dental assistant Asya Sami (D161039957) with unprofessional conduct. In September 2023, Sami allegedly did not maintain patient chart notes or ensure there was a physician or medical personnel to contact in case of an emergency during an appointment. In November 2023, Sami allegedly deleted records of her appointments with clients.

Thurston County

In February 2025 the secretary of health charged massage therapist Grayson Kane Powell (MA60886758) with unprofessional conduct. In July 2024, Powell touched a patient’s breasts without consent.

Yakima County

In February 2025 the secretary of health charged substance use disorder professional Roseanne Marie Johnson (CP61093265) with unprofessional conduct. In November 2023, Johnson allegedly failed to show for a urinalysis exam and admitted to substance use.

Out of State

California: In February 2025 the secretary of health charged substance use disorder professional trainee and agency affiliated counselor Tyler David Stewart (CO61228784, CG61092551) with unprofessional conduct. Alleged facts state that in February 2024, Stewart and a client engaged in a sexual relationship. In April 2024, Stewart was terminated from his employment after self-reporting his sexual misconduct.

Michigan: In January 2025 the Dental Quality Assurance Commission denied the dentist license of Edward Chihhua Liu (DE61574233). Liu was convicted of elder abuse in the state of California. In January 2009, May 2011 and September 2020, Liu was also involved in at least three medical malpractice settlements.

Oklahoma: In January 2025 the secretary of health charged licensed mental health counselor M. L. McBride (LH61100280) with unprofessional conduct. From April 2022 to September 2022, McBride failed to maintain properly documented patient notes resulting in 74 unfinished chart notes. McBride failed to respond to a letter of cooperation from October 2022.

Oregon: In February 2025 the Pharmacy Quality Assurance Commission released Shinta Imansjah (PH61290107) from the terms and conditions on their pharmacist license.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
