OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.
Information about health care providers is on the agency’s website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
Benton County
In February 2024 the secretary of health revoked the mental health counselor and radiologic technologist licenses of Michael Thomas Smith (LH61144228, RT60280696) with no right to reapply. In July 2023, Smith confessed to the murder of his girlfriend. Smith was charged with second-degree murder with notice of domestic violence.
King County
In March 2024 the secretary of health indefinitely suspended the certified medical assistant license of Brittany Dawn Bomann (CM60589192). Prior to petitioning for reinstatement, Bomann must enter and participate in a monitoring program. In July 2023, Bomann failed to respond to a letter of cooperation regarding allegations of being intoxicated at work.
In March 2024 the secretary of health indefinitely suspended the massage therapist license of Meijun Guo (MA60374295). Guo failed to maintain client treatment records and worked under someone else’s license despite reporting to a Department of Health investigator she had not worked in massage since October 2021.
Pierce County
In March 2024 the secretary of health suspended the substance use disorder professional trainee license of Luis Cristobal Medina (CO61033705). Medina may not petition for reinstatement for at least three years. Prior to petitioning for reinstatement, Medina must pay a $2,000 fine and pass an ethics course. In July 2023, Medina engaged in a sexual relationship with a former patient.
In March 2024 the secretary of health summarily suspended the certified nursing assistant license of Tiffany Marie Torres (NC10093963) pending further legal action. In December 2023, Torres was placed on the Department of Social and Health Services long-term care abuse and neglect registry for financially exploiting a vulnerable adult.
Spokane County
In February 2024 the secretary of health granted Anna M. Lafountain (CM61490838) a license to practice as a certified medical assistant with conditions on the license. Lafountain must complete four hours of continuing education in ethics. In August 2015, the Nursing Assistant Program entered a final order that suspended Lafountain’s certified nursing assistant license for three years and she was to complete continuing education in ethics.
Out of State
Alabama: In February 2024 the Board of Nursing (formerly the Nursing Commission) summarily suspended the registered nurse license of Marina N. Paquette (RN00172273) pending further legal action. In June 2023, the Tennessee Board of Nursing denied Paquette’s registered nurse license application for committing fraud or deceit in procuring a nurse license and for unprofessional conduct.
