OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.
Information about health care providers is on the agency’s website. Click on “Look up a health care provider license” in the “How Do I?” section of the Department of Health home page (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
Chelan County
In March 2022 the Board of Hearing and Speech suspended speech-language pathologist Tracy A. Bair Hutchins (LL00003176). Hutchins recorded treating patients but never provided the services while working at a rehabilitation facility.
Clallam County
In March 2022 the secretary of health revoked the home care aide license of John Stephen Gale (HM60604379). In 2021, Gale was convicted of assault with a deadly weapon and harassment with threats to kill in Clallam County Superior Court.
Kitsap County
In April 2022 the secretary of health suspended the substance use disorder professional trainee license of Douglas Paul Brown (CO60956432). Brown misused controlled substances.
Pierce County
In April 2022 the Nursing Commission suspended the registered nurse license of Teresa Elizabeth Keith (RN00114854) after she withdrew from a substance use monitoring program she had agreed to participate in to avoid potential discipline.
Snohomish County
In April 2022 the secretary of health suspended the home care aide license of Austin Riley McLaughlin (HM60768975) after he was convicted of domestic violence-related assault in Marysville Municipal Court.
Spokane County
In April 2022 the secretary of health suspended the registered nursing assistant license of David Y. Kolesnikov (NA60246330). Kolesnikov failed to provide scheduled medication to a patient and removed personal items from another patient’s room without permission.
In April 2022 the secretary of health denied the certified nursing assistant reactivation request for Muriel R. Snakeskin (NC60086077) and suspended her license. Snakeskin failed to comply with a 2015 agreement that required her to complete additional education and pay a fine.
Out of State
Montana: In March 2022 the Nursing Commission summarily suspended the registered nurse license of Kelley Morrow Koch (RN00087640) after the Montana State Board of Nursing revoked Koch’s registered nurse license for failing to use appropriate judgment about safe nursing practices and improperly delegated nursing care.
