OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.
Information about health care providers is on the agency’s website. Click on “Look up a health care provider license” in the “How Do I?” section of the Department of Health home page (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
Cowlitz County
In June 2022 the Dental Commission suspended the dental assistant license of Mandy Shanee Crane (D161206246) for failing to comply with the requirements of a monitoring program.
Franklin County
In June 2022 the Nursing Assistant Program and certified nursing assistant Shaelynn Marie Logozzo (NC60855746) agreed to a suspension of her license. Logozzo was convicted of two counts of assault in Benton County District Court.
Grant County
In June 2022 the secretary of health suspended the home care aide of Alison Dawn Sedlacek (HM60620759). Sedlacek told her employer that she took a patient’s hydrocodone pill and she did not respond to an investigator about the allegation.
King County
In June 2022 the Nursing Commission suspended the registered nurse license of Corina Saldivar (RN60474579) after she removed injectable hydromorphone for several patients but failed to document the administration or wastage of the medication.
In June 2022 the Substance Use Disorder Program and Stephen Robert Adkins (CP60264602), a chemical dependency professional (now substance use disorder professional), agreed to an order suspending his license. Adkins disclosed personal and emotional issues with a patient, which caused the patient emotional distress and threatened her recovery.
In June 2022 the secretary of health suspended the certified medical assistant license of Heidi Marie Vaughn (CM60366750). Vaughn was convicted of gross misdemeanor theft in Federal Way Municipal Court and Snohomish County District Court in 2015. In 2016, Vaughn was convicted of unlawful solicitation to possess a controlled substance in Pierce County Superior Court. Vaughn failed to respond to charges in relation to these convictions.
In June 2022 the secretary of health suspended for at least two years the certified nursing assistant license of Angela Lorraine Walker (NC60508680). Walker was charged with attempted identity theft and attempted theft in Kirkland Municipal Court after allegedly making unauthorized purchases with a patient’s debit card to pay her rent.
Kitsap County
In June 2022 the secretary of health suspended for at least two years the mental health counselor license of David Scott Secrest (MC60842091) because he had a romantic relationship with a patient and did not keep records of her sessions.
Pierce County
In June 2022 the secretary of health suspended the registered nursing assistant license of Alexandra Smith-Rosas (NA61172518). Smith-Rosas displayed behavior at work that made it appear she was under the influence of an intoxicant. She told a supervisor that she had taken Percocet and cocaine. Smith-Rosas did not respond to the charges.
In June 2022 the secretary of health suspended for at least 10 years the radiological technologist license of Tony Mandel Johnson (RT60040761). Johnson was convicted of two counts of child molestation in Pierce County Superior Court.
Out of State
Montana: In June 2022 the Nursing Commission suspended the registered nurse license of Kathryn S. Fowler (RN61015330) after the Montana Board of Nursing accepted the surrender of Fowler’s credential after it found that Fowler committed unprofessional conduct when she reduced intravenous fluids for an infant patient without orders from the treating physician and without contacting the treating physician.
New Mexico: In June 2022 the Board of Physical Therapy suspended the physical therapy assistant license of Daniel Kuemmerle (P160889931). In an agreed order in Minnesota, he admitted that he committed unprofessional conduct by intentionally overstating his time with clients and billing for time when work was not performed. He did not respond to Washington state charges related to the Minnesota conduct.
California: In June 2022 the secretary of health revoked the massage therapist license of Feng Shi (MA60746129). Shi’s exam score was invalidated after he was identified as one of hundreds who had cheated on the exam through the involvement of his massage school, which sold cheat sheets and faked credentials for education and training.
