OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.
Information about health care providers is on the agency’s website. Click on “Look up a health care provider license” in the “How Do I?” section of the Department of Health home page (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
Clallam County
In July 2022 the secretary of health suspended the agency affiliated counselor and substance use disorder professional trainee licenses of Daniel Thomas Clemons (CG61163703, CO61198606). Clemons failed to cooperate with an investigation. Charges stated that Clemons told a minor client how attractive she was, and he failed to keep accurate records.
In July 2022 the Nursing Commission suspended the registered nurse license of Jamie Marie Lockhart (RN60383498). Lockhart did not respond to charges that she was working as a nurse while affected by controlled substances.
Clark County
In July 2022 the Nursing Commission summarily suspended the registered nurse license of Michael Bradley LaChappelle (RN60688002). In February 2022, the Oregon State Board of Nursing suspended LaChappelle’s registered nurse credential for failing to cooperate during an investigation.
Pierce County
In July 2022 the Pharmacy Commission suspended for at least three years the pharmacy assistant license of Jonathan Wayne Cooper II (VB60744507). Cooper did not respond to charges related to his assault-domestic violence conviction in Pierce County Superior Court in December 2020.
Out of State
Idaho: In July 2022 the Nursing Commission suspended the registered nurse license of Neva Kenine Gonzalez (RN60974895) after Gonzalez did not respond to charges that she failed to report 2021 Idaho convictions to the Nursing Commission, including possession of a controlled substance, and destruction, alteration, or concealment of evidence.
