State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.

Information about health care providers is on the agency’s website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Chelan County

In September 2024 the Board of Nursing indefinitely suspended the registered nurse license of Gina Marie Huseman (RN60838074). Prior to petitioning for reinstatement, Huseman must undergo a psychological or psychiatric evaluation and comply with the evaluator’s recommendations. In July 2022, Huseman was removed from patient care by her employer pending a psychiatric evaluation. In August 2022, the Board received a complaint regarding Huseman’s ability to practice with reasonable skill and safety. Huseman did not submit a timely response to the Board’s November 2023 notice of intent to order examination, and did not comply with the Board’s March 2024 order for mental examination. In June 2024, Huseman failed to respond to a statement of charges.

King County

In September 2024 the Occupational Therapy Practice Board summarily suspended the occupational therapist license of Michelle Nicole Burkhard (OC60178454) pending further legal action. In August 2018, the Florida Board suspended her license to practice in Florida.

In September 2024 the secretary of health indefinitely suspended the certified nursing assistant license of Quamale Hakeem Daniels (NC61069521). Prior to petitioning for reinstatement, Daniels must pay a $250 fine and complete eight hours of continuing education. Daniels did not meet the terms of a November 2021 stipulation to informal disposition. Daniels also failed to respond to a statement of charges by November 2023.

Lewis County

In September 2024 the secretary of health and certified nursing assistant Austyn Madison Deel (NC60798095) entered an agreed order. The certified nursing assistant license is suspended for one year. During this time, Deel must complete an ethics and boundaries examination. In August 2023, Deel worked with a suspended license.

Lincoln County

In September 2024 the Board of Nursing summarily suspended the licensed practical nurse license of Daniel O. Sullins (LP60252289) pending further legal action. In November 2023, the Department of Social and Health Services issued a review decision and final order against Sullins for financial exploitation of a vulnerable adult. Sullins is prohibited from being employed in the care of or having unsupervised access to vulnerable adults.

Skagit County

In September 2024 the secretary of health indefinitely suspended the registered nursing assistant license of Jorak Fountian (NA61449623). Fountian did not comply with the 2024 agreement to practice with conditions by failing to participate in a monitoring program for three years.

Spokane County

In September 2024 the secretary of health indefinitely suspended the registered nursing assistant license of Addison Nichole Burrell (NA61342496). In October 2023, Burrell failed to respond to a letter of cooperation. Burrell also failed to respond to a statement of charges by June 2024.

Out of State

Hawaii: In September 2024 the Dental Quality Assurance Commission indefinitely suspended the dentist license of Karen Floyd Sheppard (DE00007852). Prior to petitioning for reinstatement, Sheppard must pay a $20,000 fine and reimburse $5,456.48 to the Commission. Sheppard must also complete 31 hours of continuing education and pass a jurisprudence examination. From March 2020 to June 2020, Sheppard failed to communicate and perform a pre-treatment examination of a patient, review records, assess a patient’s pre-orthodontic condition and maintain records, among other issues.
