OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.
Information about health care providers is on the agency’s website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
Grays Harbor County
In November 2024 the Board of Nursing charged and immediately suspended the registered nurse license of Tara L. Main (RN00150812) pending further legal action. In August 2022, the Department of Social and Health Services’ (DSHS), Division of Adult Protective Services issued a notice of initial finding for financial exploitation of a vulnerable adult. In April 2024, a review decision and final order was issued affirming the initial order and the DSHS decision that Main had financially exploited a vulnerable adult.
King County
In October 2024 the Board of Nursing indefinitely suspended the registered nurse license of Renee M. Osborne (RN00167613). Prior to petitioning for reinstatement, Osborne must contact a monitoring program and, if required, enroll in the monitoring program. In March 2023, Osborne left her shift early stating she felt unwell but was later observed in unauthorized areas. Osborne appeared intoxicated and expressed suicidal thoughts. Staff took Osborne to the emergency department where she was found with several medications taken from the hospital or patients, including morphine that she admitted she failed to waste. Osborne failed to provide a written statement to the Board as requested during the investigation.
In November 2024 the Board of Nursing indefinitely suspended the registered nurse license of Lisa Maureen Schofield (RN00078792). Between April and October 2022, Schofield made several medication administration and documentation errors, including failing to properly chart pain assessments, failing to waste or document waste of narcotic medications, and drawing medication for patients without appropriate orders.
Pierce County
In October 2024 the Board of Nursing indefinitely suspended the licensed practical nurse license of Brandon Laurence Rucker (LP61326755). Prior to petitioning for review, Rucker must undergo a psychological or psychiatric evaluation. In July 2023, the Nevada State Board of Nursing revoked Rucker's licensed practical nurse credential for failing to meet the health needs of a patient and for endangering the safety of the public, patients, clients or co-workers by making actual or implied threats of violence.
Skagit County
In October 2024 the Board of Nursing indefinitely suspended the registered nurse license of Mariana L. Guernsey (RN00142128). Prior to petitioning for review, Guernsey must contact a monitoring program and, if required, enroll in the monitoring program. In July 2020, Guernsey failed to ensure the safe discharge of a patient. In July 2021, staff reported Guernsey could not perform simple tasks. In January 2023, Guernsey entered a monitoring program and tested positive for alcohol on several occasions between April and July 2023. In May 2023, Guernsey gave discharge paperwork to the wrong patient. In August 2023, Guernsey withdrew from the monitoring program.
Spokane County
In October 2024 the Board of Nursing and Alexander Lee Blackburn (RN60525795) agreed to conditions that suspend Blackburn’s registered nurse license for at least 24 months from January 15, 2024. Between July and August 2023, Blackburn engaged in sexual misconduct with a patient.
In September 2024 the Board of Nursing charged and immediately suspended the registered nurse license of Marc Glenn Rinard (RN60130602) pending further legal action. In November 2024, the Board of Nursing permanently revoked Rinard’s registered nurse license. In August 2024, Rinard was charged with second-degree murder and first-degree assault.
Out of State
Alabama: In October 2024 the Board of Nursing and Marina N. Paquette (RN00172273) entered an agreed order that indefinity suspended Paquette’s registered nurse license. Prior to petitioning for review, Paquette must contact a monitoring program and, if required, enroll in the monitoring program. In June 2023, the Tennessee Board of Nursing denied Paquette’s application to practice as a registered nurse based on fraud or deceit in attempting to obtain a nursing license and unprofessional conduct.
Iowa: In October 2024 the Board of Nursing indefinitely suspended the registered nurse license of Dawn Marie Leach (RN60376862). In December 2022, the Massachusetts Board of Registration in Nursing placed Leach’s registered nurse license on probation.
Louisiana: In October 2024 the Board of Nursing indefinitely suspended the registered nurse and advanced registered nurse practitioner licenses of Melanie R. Baker (RN60716088, AP60744985-NP). In November 2023, the Louisiana State Board of Nursing placed Baker's advanced practice registered nurse license on probation for two years based on findings related to patient care, assessment, documentation, controlled substance prescribing issues, and the revocation of her DEA registration. In March 2024, the Alabama Board of Nursing indefinitely suspended Baker’s registered nurse license and her multistate privilege to practice based on the Louisiana disciplinary action.
Nevada: In October 2024 the Board of Nursing charged the licensed practical nurse license of Cyasia Amanda Williams (LP61465802) with unprofessional conduct. In October 2024 the Board and Williams entered an agreed order that suspended Williams licensed practical nurse license for at least 24 months. Prior to petitioning for reinstatement, Williams must complete continuing education in the area of ethical decision making. In September 2023, Williams received an erroneous payroll deposit of $199,527.85. After being notified of the error, Williams made purchases, withdrawals and transfers and did not return the funds.
Tennessee: In October 2024 the Board of Nursing and Terrie Bea Watson (AP61016247-NP) entered an agreed order that indefinitely suspended Watson’s advanced registered nurse practitioner license. Prior to petitioning for reinstatement, Watson must provide proof of completion of medical courses required by the Tennessee Board of Nursing and complete the PACE Program Ethics for Medical Professionals. In November 2023, the Tennessee Board placed Watson’s advanced practice registered nurse credential on probation for at least one year and imposed permanent prescribing restrictions based on findings that between 2013 and 2017, Watson practiced chronic pain management in a manner that fell below the standard of care.
Texas: In October 2024 the Board of Nursing charged and immediately suspended the registered nurse license of Jessica Lynne Hager-Holt (RN61291803) pending further legal action. In March 2024, the Texas Board of Nursing accepted the surrender of Hager-Holt’s registered nurse license. The Texas Board based its action on unprofessional conduct to include Hager-Holt exhibiting signs of impaired behavior while on duty and attempting to use synthetic urine during a drug screen.
