State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.

Information about health care providers is on the agency’s website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Grays Harbor

In June 2023 the Emergency Medical Services and Trauma Systems Program indefinitely suspended the emergency medical technician license of Mary Lynn Jarzabek (ES61065273). Jarzabek failed to comply with terms and conditions of the 2022 final order that was established after she was charged with unprofessional conduct in May 2022.

King County      

In June 2023 the Certified Counselor Program accepted the surrender of the certified counselor license of Samuel Conrad Zeiler (CL60168241) with agreement to never resume the practice of counseling in the state of Washington in any capacity, with no right to reinstatement. Zeiler allegedly practiced beyond the scope of his license by diagnosing nutritional or medical needs of counseling clients.

Skagit County

In June 2023 the Massage Therapist Program and Shannon Jean McCain (MA60921442) agreed to an indefinite suspension of her massage therapist license. McCain may petition for reinstatement after paying a fine of $2,000 and completing 12 hours of continuing education in the areas of law and ethics. Between January 2018 and November 2021, McCain taught and supervised students without an active license, in addition to, failing to keep patient records or teach students recordkeeping, and did not maintain attendance records for any student.

Out of State

Georgia: In June 2023 the Board of Nursing (formerly the Nursing Commission) indefinitely suspended the licensed practical nurse license of Shawntea G. Hamilton (LP61033790). Hamilton may petition for reinstatement after provision of evidence that her nursing credential in Michigan state has been reinstated. In April 2021, Hamilton’s licensed practical nurse license was restricted by the Michigan Board of Nursing Disciplinary Subcommittee for misconduct.
