OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.
Information about health care providers is on the agency’s website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
Benton County
In August 2023 the Home Care Aide Certification Program and Stacey Renee Bennett (HM60993138) agreed to the suspension of her certified home care aide license. Bennett may not petition for reinstatement for at least one year. Prior to petitioning for reinstatement, Bennett must successfully complete an ethics examination.
King County
In August 2023 the Pharmacy Quality Assurance Commission determined that the pharmacy intern registration of Kelsey T. Le (IR61056559) is inoperable. As of May 2023, Le was no longer enrolled in a pharmacy program nor making progress towards graduation, which is necessary to maintain the registration.
Snohomish County
In August 2023 the sectary of health revoked the home care aide license of Danielle Marie Evans (HM60901079). In April 2023, Evans was placed on a registry as a person who is disqualified from caring for vulnerable adults, juveniles, and children. Placement on this list prohibits certification as a home care aide. Between July and September 2022, Evans financially exploited a vulnerable adult.
Out of State
Oregon: In August 2023 the Department of Health rescinded the massage therapist license of Baixue Ye (MA61017731). The license was issued erroneously in February 2020 prior to the Florida Board of Massage Therapy rescinding the approval of the World Beauty Academy where Ye received their education.
Oregon: In August 2023 the Board of Nursing (formerly the Nursing Commission) indefinitely suspended the registered nurse license of Shannon M. Ryan (RN00160616). Ryan failed to comply with the terms and conditions of a 2020 Agreed Order by failing to check in with the monitoring program on three separate dates, missing a non-random test, failing to report a relapse to the monitoring program, and by officially withdrawing from the monitoring program.
