OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.
Information about health care providers is on the agency’s website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
Benton County
In August 2023 the secretary of health indefinitely suspended the certified nursing assistant license of Guadalupe Gutierrez (NC60151060). Gutierrez failed to comply with the terms and conditions of a 2023 agreed order.
King County
In September 2023 the Board of Nursing (formerly the Nursing Commission) and Mary Jane S. Wayman (RN60089672) agreed to an indefinite suspension of her registered nurse license and modification of a 2022 agreed order on reinstatement. Prior to petitioning for reinstatement, Wayman must contact a monitoring program and, if required, enroll in the monitoring program. If reinstatement is granted, Wayman must successfully complete a refresher program.
Pierce County
In September 2023 the Marriage and Family Therapist Program and Timothy James Carver (LF61110001) agreed to the suspension of his marriage and family therapist license. Carver cannot petition for reinstatement for at least five years, and he must pay a $5,000 fine. Prior to petitioning for reinstatement, Carver must attend and pass an ethics course. If the license is reinstated, it would be subject to probation among other conditions. Between July 2021 and February 2022, Carver initiated a sexual relationship with a patient he was providing therapy services to. By March 2022, Carver ended the professional relationship to pursue a sexual relationship with the patient.
Walla Walla
In August 2023 the secretary of health indefinitely suspended the clinical social worker and certified nursing assistant licenses of Rhaejon Monique Ongers (NC60951019, LW00009729). Ongers failed to comply with the terms and conditions of a 2022 agreed order.
Out of State
Oklahoma: In September 2023 the Board of Nursing (formerly the Nursing Commission) indefinitely suspended the registered nurse license of Tatiana Maria Burke (RN60936657). Burke may petition for reinstatement. In September 2021, the Oklahoma Board of Nursing reprimanded Burke’s multistate privilege to practice in Oklahoma and required Burke to complete certain terms and conditions including a nurse refresher course. The Oklahoma Board made findings of unprofessional conduct including inaccurate recording, falsifying or altering patient records, conduct detrimental to the public interest, jeopardizing the life, health, or safety of a patient by the unauthorized altering of medications, failing to utilize appropriate judgment in administering safe nursing practice or patient care, and failing to exercise technical competence in carrying out nursing or patient care.
Mississippi: In September 2023 the Dental Quality Assurance Commission suspended the dentist license of David Barclay Dowling (DE00004069) pending further legal action. In October 2022, Dowling surrendered his license in California in response to an accusation of negligence, patient abandonment, violations of the telehealth statute, among other actions.
