OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.
Information about health care providers is on the agency’s website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
Benton County
In December 2023 the Nursing Assistant Program summarily suspended the registered nursing assistant license of Omar Adi (NA60206508) pending further legal action. In January 2023, Adi was placed on the Department of Social and Health Services long-term care abuse and neglect registry for neglecting a vulnerable adult.
King County
In December 2023 the Board of Nursing (formerly the Nursing Commission) indefinitely suspended the registered nurse license of Bruce Everett Beauchamp (RN60585595). Prior to petitioning for reinstatement, Beauchamp must complete 22 hours of continuing education in the areas of medication errors, time management, and communication skills. Beauchamp must also submit a written report on pediatric diabetes and insulin administration. On two occasions in January 2022, Beauchamp administered incorrect dosages of medication to a minor patient and failed to report the medication errors to his supervisor.
Pierce County
In November 2023 the Dental Quality Assurance Commission indefinitely suspended the dental assistant license of Jasmine Lavina Naniloa Kananiokahome Caminos (D160575758) for failure to respond. Prior to petitioning for reinstatement, Caminos must respond to the letter of cooperation regarding allegations of fraud and drug diversion and follow her monitoring program contract from 2022.
Snohomish County
In December 2023 the Board of Nursing (formerly the Nursing Commission) suspended the registered nurse license of Jeanne Ang Rather (RN60427259) for at least five years. Rather failed to respond to a statement of charges regarding her convictions of bank fraud and aggravated identity theft in the United States District Court, Western District of Washington.
Spokane County
In December 2023 the secretary of health indefinitely suspended the registered nursing assistant and home care aide licenses of Nikita Dawn Gilbert (NA60753512, HM60891249). Prior to reinstatement, Gilbert must respond to the allegations in the letter of cooperation against her. In February 2023, Gilbert failed to respond to a letter of cooperation regarding allegations of financial exploitation of a patient.
