State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.

Information about health care providers is on the agency’s website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Asotin County

In April 2024 the Board of Nursing (formerly the Nursing Commission) suspended the registered nurse license of John Edward Hulsey (RN60859412). Hulsey may not petition for reinstatement for at least five years. Prior to petitioning for reinstatement, Hulsey must complete any court-ordered treatment plan and must complete a psycho-sexual evaluation. In December 2023, Hulsey was charged with first-degree voyeurism, a class C felony.

Cowlitz County

In April 2024 the Board of Nursing (formerly the Nursing Commission) summarily suspended the registered nurse license of Michael J. Ross (RN00150654) pending further legal action. In January 2024, the Arizona Board of Nursing revoked Ross’s registered nurse license for unprofessional conduct. In March 2023, Ross was charged with nine counts of dealing in depictions of minor engaged in sexually explicit conduct in the second degree, 20 counts of possession of depictions of minor engaged in sexually explicit conduct in the second degree, and seven counts of possession of depictions of minor engaged in sexually explicit conduct in the first degree, all class B felonies. Ross was also charged with attempted tampering with physical evidence, a misdemeanor.

King County

In April 2024 the Board of Nursing (formerly the Nursing Commission) indefinitely suspended the licensed practical nurse license of Monica A. Davis (LP60066246). Prior to petitioning for reinstatement, Davis must provide a written statement addressing the complaint allegations and complete a continuing education program. Between approximately October 2022 and December 2022, Davis had a romantic relationship with an incarcerated individual at the correctional facility where she worked.

Kitsap County

In April 2024 the secretary of health and Alion Anthony Mansfield (CG60972601, CP60686667, CO60679949) agreed to the suspension of his agency affiliated counselor, substance use disorder professional trainee, and substance use disorder professional licenses for at least five years. Prior to petitioning for reinstatement, Mansfield must pay a $1,000 fine and pass an ethics course. From October 2022 to February 2023, Mansfield had a sexual relationship with a patient.

Pierce County

In April 2024 the secretary of health suspended the certified nursing assistant license of Tiffany Marie Torres (NC10093963) for at least two years. Prior to petitioning for reinstatement, Torres must pay a $250 fine and must complete an ethics examination. In December 2023, Torres was placed on the Department of Social and Health Services’ Adult Protective Services long-term care, abuse and neglect registry and is prohibited from being employed in the care of or having unsupervised access to vulnerable adults.

Out of State

Colorado: In April 2024 the Board of Nursing (formerly the Nursing Commission) summarily suspended the registered nurse and advanced registered nurse practitioner licenses of Kristen Rose Bolling (AP60740480-NP, RN60740478) pending further legal action. In December 2023, the Wyoming Board of Nursing summarily suspended Bolling’s advanced practice registered nurse license for unprofessional conduct based on Bolling’s plea of guilty to conspiracy to commit health care fraud in the state of Idaho.

Florida: In April 2024 the Board of Nursing (formerly the Nursing Commission) suspended the registered nurse and advanced registered nurse practitioner licenses of Elizabeth Mercedes Hernandez (AP60931942-NP, RN60998655). Hernandez may not petition for reinstatement for at least 20 years. In December 2023, Hernandez was convicted of one count of conspiracy to commit health care fraud and wire fraud, four counts of health care fraud, and three counts of false statements relating to health care matters and sentenced to a total of 240 months imprisonment.
