Public and Private K-12 Schools Immunization Reporting

Washington State law requires all public and private schools with any students in kindergarten through 12th grades to complete and file an Immunization Status Report to the Department of Health. RCW 28A.210.110 and WAC 246-105-060.

Reporting will open on November 1st and is due by December 1st.

The information on this webpage is for reporting the immunization status of students in transition to kindergarten through 12th grades. Preschool and Child Care reporting information is available at Child Care and Early Learning Immunization Reporting.

Reporting Options

The annual report data may be submitted using one of the following methods:

School Module – Preferred Reporting Method

An active user of the School Module (who keeps the Roster up to date and enters missing immunization dates and exemptions), does not have to file a report. The report will be drawn from School Module data. For more information about the School Module, please visit: www.doh.wa.gov/schoolmodule.

Direct Reporting

We will be sending information about this year's report via our Listserve by November 1st. To sign up for updates to the immunization requirements and reporting, enter your email, click the submit button, and select the school list option from the immunization choices given.

By November 1st, an email will be sent to the reporter on record or school administrator (principal or head of school) regarding the annual immunization report. The email will include an instruction worksheet and training documents. It will also include a link to the online reporting tool. This link is unique to the school and should not be used for other schools. If not the reporter on record, the school administrators may forward the email to the person designated to complete the report. If the reporter on record or school administrator did not receive their reporting email please notify DOH by sending an email to OICPSchools@doh.wa.gov.

How to Report

  1. Use the K-12 School Annual Report Worksheet (PDF) to gather the information you need for the report.

  3. Use the unique link (info above) emailed to reporter of record on November 1st, to open the survey and complete the report.

The online report tool or reporting through the School Module (see above) are the only reporting methods that will be accepted. All reports submitted with the online reporting tool MUST BE SUBMITTED BY DECEMBER 1st.
