Reciprocal certification is provided only to individuals who have successfully completed the Oregon State sponsored Basic Decontamination Worker Course and submitted the completed Washington State Department of Health's Clandestine Drug Lab Decontamination Worker Application within 60 days of completing Oregon's Worker Course.
Step 1
Read the Reciprocal Worker Certification Application (PDF) to make sure you understand the requirements for worker reciprocity certification.
Step 2
Complete the application form.
Step 3
- Provide current copies of your forty-hour Hazardous Waste Operations (HAZWOPER) training. HAZWOPER classes are offered through private companies and community colleges. If 40-hour HAZWOPER was taken over one year ago, also submit a copy of your current 8-hour annual HAZWOPER refresher certificate.
- Provide a copy of the CDL Worker Course completion certificate, with class date and exam score 70% or higher issued by the state of Oregon.
- Provide a copy of a valid state issued driver’s license.
Step 4
Submit the application for reciprocity within 60 days of successful completion or CDL worker course.
Reciprocal application must be received before current certification expires or application will not be acted upon, and the worker course must be retaken. If it has been more than sixty days but less than a year since completing the worker course, the applicant may be required to take a department administered test that must be passed with a score of 70% or higher.
