The following steps must be completed to become a certified clandestine drug lab decontamination contractor. Contractor certification lasts for 1 year. To determine the exact application fee, see the Contractor Certification Application Form below.
Step 1
Be licensed, bonded, and insured as a Washington State General Contractor through L&I.
Step 2
Have a minimum of two Department of Health certified drug lab decontamination employees, at least one of which must be a certified supervisor.
Step 3
Submit a Contractor Certification Application Form (PDF).
Mail the form, fee, and the additional required information to:
