For questions about reporting requirements, adverse event definitions, check-in survey, quarterly reports or other electronic or online tools, or for other general program inquiries and adverse event notification submissions and updates to facility contact information.

The Department of Health does not recommend sending Root Cause Analysis and Action Plans electronically. We are not able to ensure confidentiality is maintained via non-secure email.
