The coordinated quality improvement application process includes submitting:
- A completed Application Form (Word)
- The submitted application must have the following:
- A table of contents that clearly specifies where each component of WAC 246-50-020 is located within the CQIP plan and indicates the page number.
- A written plan that meets the minimum requirements of a department-approved CQIP plan. The areas marked with a (?) and in bold type on the CQIP Guidelines (PDF)* are the minimum required components specified in WAC 246-50-020 and must be described in detail within the CQIP plan
- If the plan is a modification indicate in the cover letter the modification from the original plan.
- Send your completed application and application plan to cqip@doh.wa.gov:
- The department will review the submitted application and fee when they are both received.
- The department will send an approval letter once it is reviewed and approved by the coordinated quality improvement program office. However if the reviewer needs more information the reviewer will contact the health care entity for additional information.
- Send your fee to the (please note that your check must note the applicant name and the Revenue Code of 05 97 912040 on the check or money order:
