Each year, providers must renew their Provider Agreement for the Receipt of Publicly Supplied Vaccine. In May 2013, providers began using the Washington State Immunization Information System (IIS) to complete their annual enrollment. Your local health jurisdiction and the State Department of Health will notify you when it is time for you to renew.

Initial Enrollment
Please note: Under advice of counsel, new enrollment of a Direct Care Practice into the Childhood Vaccine Program is temporarily on pause pending review of state rules related to these practices and the Childhood Vaccine Program. We will share more information as it is available and when enrollment of Direct Care Practices resumes.
